Bob can you help get the InDesign EPUB forum listed in Related forums?
Weird! I haven't seen that before in an EPUB.
Version of InDesign? Format of images? If you create a new doc, place a "normal" image (tiff, psd, ai), and export it to EPUB, does it appear properly?

Using InDesign CC 2014 and it's updated ( I've even tried InDesign CC). It's a combination of jpegs and PSDs. Tried creating a new document and exporting them to EPUB as you suggested and this is where things get interesting...

What are you using to check your EPUB. Does is pass the epub check? Are you exporting Fixed Layout or Reflowable?
If you can share a sample epub with the problem we can take a look at it. You can use dropbox.com or similar.
By the way, what color space are the images?
We'll need the image since that's the crucial element to test or you can share the actual epub sample.
Thats so strange that you can export the image but I can't. Settings are default. Haven't really changed anything. Exporting with epub 2.01 too. Also tried epub 3.0 but same issue.

If other people can export it fine but you can't, then I would suggest some troubleshooting of your InDesign install.
Have you tried rebuilding your preferences and then re-exporting?
Have you made sure you're not using any third-party plugins that automatically load with InDesign?
Assuming you've done those two tests and are still having a problem, then try it on a test account on your computer. Package the InDesign file and put it on a thumbdrive (or I guess you could just download the files from the link above) and then log out of your account on your computer, and log in again to a fresh new account with admin privileges. (If you haven't created one yet, you should do that first before logging out ... lol)
When you log in to the new account, all the apps are available to you but none of your other account's files or user settings. So open InDesign, download/copy over the packaged file, and open the layout and export. See if that makes any difference.

After looking at your epub file I cannot see any difference regarding the image CSS styles between yours and mine. The only difference is that you are are using a Mac and I'm on Windows 8.1. Follow AnnMarie's advice and hopefully you can find a solution.
I did a reinstall of indesign and that fixed the problem. Thanks!
