I have seen this problem before and I can not figure it out so I gave up because it was only happening on my small test prints on 5 X 7. Now I have a project where I am printing larger images on a 11 X 17 sheet of photo paper and it keeps cutting off my image. Arghhhhh.
Here is a link to a screen shot of my settings and also an actual cmeraphone picture of the print itself. Why oh why is it cutting off the image?. Please let me know if I have something wrong.

This is just a guess, but since the printer can print up to 13-inches, maybe the printer thinks you have 13-inch paper installed because of a setting somewhere outside of LR in the printer settings so is calculating the wrong print-head position for printing. You've only shown us the LR settings, not the printer-driver settings.
It could also be that the paper is installed shifted over and the printer doesn't realize this and is printing as if the paper is installed with a different alignment. I don't have this printer so am not sure if this is a possibility or not.
Also, instead of making an HTML page on dropbox, just copy/paste the screenshots into the text box on the web version of the forums, then type explanatory text above and below the screenshots.
