1. Re: The dropdown calendar works in the Test mode, but when I create the form the dropdown calendar icon does not appear on the form next to the date field. How can I get this feature to work properly?

Need more information. The drop down calendars are a custom JavaScript that has been developed by a number of different people and use many different methods so without which one and how you implemented it ion the form it hard to determine what is going on. -

tneves Sep 11, 2014 11:15 AM (in response to GKaiseril)Thanks for the response. I am using Adobe: Forms Central to create the form and once the form is created, I am using Adobe Acrobat XI Pro. Does that help?

George_Johnson Sep 11, 2014 4:22 PM (in response to tneves)The calendar that's available with web forms in FormsCentral is not available when converted to a PDF form. Adobe does not provide a date picker for AcroForms, but others have created one using a collection of fields as well as a custom dialog.
