What InDesign version?
What OS name and version?
What type of fonts you are using in InDeisgn document. Are they double bytes fonts.
It would be great if you post a screen shot of problem.

I've sent you a screenshot of what it looks like when trying to convert to interactive pdf.
Funny thing was it was working fine until a few tweaks and then decided to do its own thing.
I've created loads of forms before through Indesign but this error is bizzare
Hm, could you provide an indd file with the form together with the exported PDF for downloading at a dropbox account (or a similar service), that we can look into it? Seems, that all the additional text and the company logo is not travelling over to PDF. Strange. Could it be that all missing elements are on a none-printing layer? (Just to rule out something obviously)

Hm, I had a look at your InDesign document.
The culprit seems to be the gray rectangle in the background.
That should be no formfield. Its name is "Text Field 17". Convert it back to a regular rectangle…
Without that (and removing all occurances of overset text: superfluous paragraph signs) I had no problems at all.
