Please try to run EM as administrator then install the extension.
1. Run "/Applications/Utilities/Terminal.app".
2. Input the following command then press "Return" key. Don't miss the double quotation marks.
sudo "/Applications/Adobe Extension Manager CC/Adobe Extension Manager CC.app/Contents/MacOS/Adobe Extension Manager CC" (for CC)
sudo "/Applications/Adobe Extension Manager CS6/Adobe Extension Manager CS6.app/Contents/MacOS/Adobe Extension Manager CS6" (for CS6)
3. Input password of administrative user then press "Return" key.

I wonder why this happens, this can't be normal procedure.
There may be some files/folders which have incorrect permission. Normally you have to run it as admin only one time. Now you can run it as normal.
I see, but I've tried to run it again normally and the same thing happened. I have to use the terminal for it to work. Thanks again.
I have the same problem and tried your fix but the same thing happened... it hangs 3/4 way through loading extensions. I'm using OS X 10.10.1 and PS CS6 and other ideas?
I tried again this time including the sudo. sudo "/Applications/Adobe Extension Manager CS6/Adobe Extension Manager CS6.app/Contents/MacOS/Adobe Extension Manager CS6"
It came up with an error message and asked for the passwprd I added that and it worked. IT still won't add any new extensions but I got the old ones back.
