I am currently running Photoshop CS4 on Maverick (10.9.4). It used to work but, increasingly, I am having problems (files don't fully open, printing problems). I've tried re-installing CS4 to no avail. I want to do the least upgrade (spend the least money) necessary to get back to compatibility... Will CS5 be sufficient?

We have photoshop cs4 on mavericks 10.9.5 and it always seems to work okay, though we probably don't use cs4 as much as you, since we have cs6 and photoshop cc/cc 2014 as well.
(there are a few glitches here and there, but nothing major)
Have you tried resettings the photoshop cs4 preferences?
How to reset the photoshop preferences (mac)
Press and hold the Shift+Command+Option keys while starting photoshop cs4
Keep holding the keys down until you get a dialog asking if you want to delete the adobe photoshop settings file
Press Yes because you do.
Have you repaired Disk Permissions using the Disk Utility in Applications>Utilities?
