    Writing details from a PDF file when a budget director submits it

      We have a PDF file which is a form having fields like

      Budget name
      Budget category
      Budget number
      Date

      and few more fields. There is also a Submit button(form field) in it for the Budget Director along with a check box(form field) and a place where he can sign(form field like text box) using the
      Digital ID in Acrobat PDF. The digital ID will have his name, organization name, his title, e-mail address etc.
      When he clicks the button, the form will be saved, then attached to a e-mail client(he uses MS-Outlook), with the e-mail address of Executive Director of Budget.

      The budget director will fill the form, check the box next to his title, sign it using his digital ID, then click the Submit button after which it
      will be attached to a e-mail client(he uses MS-Outlook), with the e-mail address of Executive Director of Budget.

      At the end of the year, the budget office director wants to track how many times, he has filled the form, for which budget name, budget category, budget number, date
      checked the check box, signed it using his digital ID and sent it to the Executive Director of Budget.

      I understand using Javascript in PDF, when the submit button is clicked, it can be determined if the check box for Budget Director is checked and if so does the digital ID belong to the 
      Budget Director.

      Then, information like budget name, budget category, budget number, date can be extracted from the form, written to a text file or spreadsheet on a server using PHP which can be called using Javascript.

      The implementation outline would be

      1. Using JavaScript, when the Submit button next to Budget director is clicked, check if the check box for Budget Director is checked and if so does the digital ID belong to the 
      Budget Director.

      2. When those conditions are applicable, using JavaScript, create an XmlHttpRequest object which dispatches off a GET/POST method to a PHP page which has a PHP function.

      3. Form fields(Budget name, Budget category, Budget number, Date) from the PDF can be extracted, then stored in variables of JavaScript
      which can be passed to the PHP page which can update the text file or spreadsheet.

      4. At the end of year, when the budget director wants to know how many budget requests, he sent to the Executive Director of Budget, he can
      open the text file or spreadsheet from the server and find out.

      Questions are:

      1. Is my thinking on the right track?

      2. To verify in Javascript, that the Adobe digital ID belongs to budget director, what should be done? Should the budget director store the new digital ID using
      Windows certificate store or doing it in a New PKCS#12 digital ID file be easier to check in JavaScript that the digital ID belongs to the budget director?

      3. If confirming in JavaScript that the digital ID belongs to the budget director is difficult, can the remaining part be done as described?

      4. Are there other ways to implement what is needed?

      Any other suggestions would be appreciated.
