We have a PDF file which is a form having fields like
and few more fields. There is also a Submit button(form field) in it for the Budget Director along with a check box(form field) and a place where he can sign(form field like text box) using the
Digital ID in Acrobat PDF. The digital ID will have his name, organization name, his title, e-mail address etc.
When he clicks the button, the form will be saved, then attached to a e-mail client(he uses MS-Outlook), with the e-mail address of Executive Director of Budget.
The budget director will fill the form, check the box next to his title, sign it using his digital ID, then click the Submit button after which it
will be attached to a e-mail client(he uses MS-Outlook), with the e-mail address of Executive Director of Budget.
At the end of the year, the budget office director wants to track how many times, he has filled the form, for which budget name, budget category, budget number, date
checked the check box, signed it using his digital ID and sent it to the Executive Director of Budget.
The implementation outline would be
which can be passed to the PHP page which can update the text file or spreadsheet.

open the text file or spreadsheet from the server and find out.
