Table of contents / nav bar for interactive PDF.

      ID CS6.

I'm making a catalog, and I'd like to have a clickable nav bar / table of contents that appears on every page of a 40-50 page interactive pdf.  A couple times a year, pages will be added, so ideally the links will update if a page is added, but it's not 100% necessary.

What is the wisest way to set this up?  From my interpretation, unique buttons for all 40-50 page links on each page would be an organizational nightmare.  I'm ok with no frills hyperlinks.  Can anybody point me in the direction of the best combination of functionality/flexibility/aesthetics?  I'm assuming there is some sort of native automation for this, but not sure of the exact syntax I should be searching for.
