I use CS6 and have never had a problem with synchronizing photos in ACR. Just to verify, you are talking about syncing setting you applied on on photo to the rest of the selected photos, right?
Yes. It used to work just fine, but no longer. Any ideas where to start?
First, what version of ACR are you using? Also, what camera?
I better start at the beginning:
I use a MAC OS 10.8. When I use CS5 (needed because it has "picture package") ACR version is 22.214.171.1249
In CS 6 ACR is version 126.96.36.199
Cameras: Canon EOS 1D Mark IV and Nikon D300S, plus several other models of both cameras.
First, try purging the cache for Bridge and Camera Raw. If that doesn't work, also try clearing all Camera Raw settings on the photos and starting over. If that doesn't work, press Command+Option+Shift the exact instant you press "Open in Camera Raw." It will ask if you want to reset Camera Raw - select yes.

Thanks, purging the cache for Bridge and Camera raw worked for the new images I processed so far; I did not have to go to the 2nd and third step.
I hope this will continue to work, because as I mentioned originally the problem with "synchronizing all" was intermittent.
For the 3rd step - "CommandOptionShift" - will this reset my preferences? If so, for everything in Photoshop or just Bridge and Camera Raw? Do I need to save my preferences somewhere?

Awesome! Thank you for checking back.. Hope it keeps going good. No, the reset won't do anything to Photoshop. If you use the same keys on start up of Photoshop, then it will be reset.
