How do I count and record my checked boxes on a PDF?

I assume the only way is to use JavaScript.  Sadly I have never used JavaScript and as a result, I will need very specific directions on how to do this.  I have been learning about it, and understand the basics of it, but I have yet to apply a script to a PDF and as a result will need a good tutorial for the first one.
