VivekPassan Nov 1, 2014 11:49 PM (in response to colshashi)Hello,
You can easily achieve this by moving the field next to the label.
Select the Desired form field and move (Drag and drop) it to the desired location in the Form container.
Before performing the Drag drop action please make sure Form field is selected on the other container.

VivekPassan Nov 2, 2014 4:27 PM (in response to colshashi)Hello.
It seems that you wish to create a list of items to be checked by visitor in the form and you want to create multiple check boxes.
Lets assume you want to create 3 list items with a check box. Simply go to form Options and Click on Check box option 3 times and you will observe that three check boxes have been created. Now you can edit check box label as per your requirement by double clicking it.
As of now we do not have the option to insert Date field in Muse Form Widget.

VivekPassan Nov 2, 2014 4:48 PM (in response to colshashi)Hello,
At this moment we do not have this functionality in Muse Form.
I would suggest you to go to the link below and voe for this idea.
https://forums.adobe.com/ideas/2329
As this point I would suggest you to create a form of your requirement at http://www.jotform.com/.
Once the form is created, generate the embedded code for the form and place it in Muse by Object > Insert HTML.

Andrew Hushbeck Nov 2, 2014 10:54 PM (in response to colshashi)Hi colshashi,
That's the only demo site I have, but if you are curious about functionality, or would like to see something in particular, feel free to let me know and I can try and make sure you know what it can do. You can also check out a screencast of it in action if you'd like to see how it is used. And of course, if you have any questions feel free to ask
Hope that helps,
Andrew
