Form problems

I have a form that I have created from a WORD file. All field worked for the editor. When sent back to me it appears as all the fields are blank until i click on the individual field to view what was filled out. It will not let me view the entire form filled out nor will it allow me to print the entire form filled out. How do i fix this?

The form was most likely filled using the buggy application Apple Preview,
which corrupted it.
To fix it you can use this utility:

A more permanent solution is to tell the users to stick to Adobe Reader,
instead of Preview.

This did not work for me. I am using a MAC but the fix did not do anything. I have asked the user to tell me what they used to fill it out and i guess i will have to wait to find that out.
