Photoshop Extended (Stand Alone - not CC) for Mac - Student Version.
The downloaded archive file has only Setup.exe, which won't execute on my
Mac. A .dmg file isn't included.

So you do not want the subscription, but a permanent license at educational pricing. That would be Photoshop CS6 Mac.

You will have to contact your school for an educational purchase. The Students and Teachers section only offers Creative Cloud.
