On the 2/12/14 I completed an on-line form for the Care Quality Commission using formscentral, which I saved on line and received an email with a new link to the form. When I came to look at the form again yesterday there is no trace of this email anywhere, not in inbox, spam or trash, even though there are other messages in these folders from the same date. It was a lengthy questionnaire and took a considerable amount of time to complete, is there any way you can re-send the email ?

I would check with your e-mail administrator, to see if the e-mail can be recovered from your mail server backups.
Also, if you/your company use G-mail as your e-mail platform, note that they sometimes "helpfully" file e-mail for you in unexpected places.
Thanks Brian. I have already contacted Yahoo regarding resetting the account to an earlier date, just waiting to here from them as well.
