Captivate 6 reporting to CourseMill LMS each question as a separate attempt

I'm using Captivate 6 and we just began using CourseMill as our LMS.  When I look at interactions, someone who took the assessment one time, shows there were multiple attempts.  Each test question is reporting a separate attempt for the course (See below).  They only made one attempt per question, so the interaction should show 1 attempt and a score of 100%.  Does anyone know a fix for this?

Can you upload the course to SCORM cloud (you can do some tests for free) to check? If it works OK there, problem is not with Captivate but with the LMS.
