Troubles with image processing and banding

I have several thousand files which need to be processed. I've generated an action command which, when utilized manually, appears to work. However, when I try to run a file process I receive an error message indicating that Photoshop was unable to process the files at the end of the processing attempt. The files it does kick out have severe banding where a gradient mask is supposed to fade the edges of the image to a light grey (achieved with two separate layers and varying gradients). In the action sequence I have already changed modes to 16 bits and added a rasterized color layer and added noise twice to it, and then changed the opacity and fill to 19% and 1% respectively. I'm wondering why I can do this and then save it into a jpeg and not see banding, whereas when the files I process go through, they have banding.
