In what version did you make the original file?
How did you generate the cover image?
How did you export it?
What are the export settings?
Which version of ADE do you use?
And are you aware that every reader shows EPUB different? Most reliable is Apple iBooks.

1: Updated InDesign CC
2: I generated by choosing an image choosing image.
3: Exported as epub 2.0.1 to reflow epub

6: Yea, I am trying to make an export that behaves well overall. The las image is from a another epub, where the cover is fully shown in the interface. I cannot understand why i cant get the same result. But in relation to your question, iBooks is super reliable, and always shows consistent results. But i know its possible to get it to work in ADE 3.0 as well. I just dont know how.
What I prefer to do is, that I place a PDF/X-4 (with RGB images) at the first page created from the cover and use the first page as cover file. So I get always stable results.
