This totally depends on the kind of shot you have and the movement in it.
If your camera and object is moving you need a 3d camera track and an object track for the hand. If the camera is static and only the hand and arm is moving you might get away with just doing a 1 or 2 point track inside of AFX or planar track in Mocha (which is bundled with AFX).
Maybe just search for "inserting object into live action plate" in your web search engine and you probably find hundreds of tutorials and shots that are similar to yours. There you will se what the artist suggests.
