As a matter of interest (not that this will particularly help your current issue), why did you choose fixed layout ePub. Maybe your book would be better with a reflowable text ePub?
Have you tried a short trial document or maybe use one of Anne-Marie's downloadable samples to see if you're getting the same issue. If so, you probably have something like an application corruption. What does it state when you validate your ePub?
By the way, there is a forum for InDesign ePub.

How are you reading this EPUB? Many readers are incapable of properly displaying them.
iBooks on iOS or OSX or Readium for Chrome are highly recommended and Adobe Digital Edition version 4 isn’t too bad either.
It sounds to me like you’re trying to view it using something else.
Hi, Derek: I've chosen Fixed Layout because the book is laced with a lot of historic photographs and with particular formatting. I designed it for print, and it was published that way, and I"m pleased with the result; but now I'd like it to be an e-book-- and to retain its look. I'd also like to figure out fixed-layout epub for similar projects. I'll look for the InDesign e-pub forum. Thanks very much for this!
