Why can I not delete/update links?

I am having difficulty managing some troublesome links in my InDesign document. I had a link that I changed outside of InDesign. When I went to update the link in the Links panel, it created a duplicate instead of refreshing the altered link. The original link is now showing as altered (yellow exclamation symbol) but does not exist on any pages and cannot be found within the document. I have checked off the art board, deleted the "new" link that InDesign created, even deleted all objects on all pages and master pages just for kicks. I cannot seem to find this link. It has happened 3 times in this one document (see image). I am able to "relink" the file and, when trying to select a different file for troubleshooting reasons, it doesn't seem to change the file listed in the Links panel.

Pardon my blurring of the image. Ive done this out of respect for my employer.

I am working in InDesign CS6 on an iMac running 10.9.5.
