What version of InDesign is this?
This is the comprehensive list of everywhere In-Tools components are installed:
/Library/Application Support/Adobe/CS[x.x]ServiceManager/extensions/WorldToolsPlus (or WorldToolsBasic)
/Library/Application Support/Adobe/CEPServiceManager4/extensions/WorldToolsPlus (or WorldToolsBasic)
InDesign CC 2014:
/Library/Application Support/Adobe/CEP/extensions/WorldToolsPlus (or WorldToolsBasic)
Note: The /Library locations are system wide (and not user specific).

I pinged Harbs and he's on the case (for those who don't know already Harbs is the mastermind behind In-Tools, and one of our regular contributors).
Fast day, today. I was one minute late after I looked up the web address.
FWIW, CC 2015 has been a HUGE headache for all third party developers.
Adobe has blurred the distinction between different software versions too much and products can be partially installed into CC 2015 automatically. We have no method right now to allow users to pick which version they want to install extensions into.
To top it all off, Extension Manager is not really supported any more, and it's highly unreliable right now. (Even more than it's been to date.)
We've actually started work on a cooperative open source installation solution, but it's a lot of work and it'll be a while before it's ready for prime time.

Thanks for the quick response and answers to my question. Indesign CC15 is the program. I've removed all the listed items and the entry still remains in the menus. I do recall that this problem seemed to appear after I upgraded to the latest ID version. The thing doesn't seem to be hurting performance or causing crashes. It's just bothersome seeing that appearing in the menus. I can certainly live with it until a solution is developed.
Have you tried uninstalling CC 2015 ( and maybe running the cleaner tool) then reinstalling?
If the extension is really gone, trashing your preferences will get rid of the menu.
YES!!! That did it. Thank you Harbs! This info should be added to the uninstall process for In_tools.
