Check the EXIF data in a file from the camera with some other tool. Maybe the camera does not write it correctly with this lens.
for example in EXIF-reader (http://regex.info/exif.cgi?imgurl=http%)
there is indicated: HD PENTAX-DA 645 28-45mm F4.5 ED AW SR Shot at 28 mm
So I think the lens is wrote in the EXIF, but LR cannot find it automaticly, only when I give in PENTAX in the lens-corrections-module LR can find the profile (not automaticly as all other lenses by Pentax!).
Is there a HD and a non HD lens of your typoe available, then the same applies to your lens.
I am currently not near my LR computer and I can not check the support myself.
No, there is only one Version of the (Mid-Format) DA645 28-45mm ED AW SR. It is na new lens, for about good half an year in the market! There was never before a Pentax MId-Format lens with 28-45mm!
The right profile is in LR 5.7, but LR cannot find it automaticly! And by sorting fotos in the bibliothek-modus with meta-dates (lens), there is indicated "unknown lens". How sayed, only with this Pentax 645 lens. In LR5.7 specifications, the lens is listed!
Would you mind sharing a photo file using some file sharing service?
Maybe somebody here can have a look at it ... or I will be but I will not be able before tomorrow, unfortunately.

Solution to have the lens automatically detected:
1) Select a raw image file shot with the lens in LR.
2) In the Develop module Lens corrections > Profile tab select the proper Make and Model lens profile.
3) Click on Setup and select 'Save New Lens Profile Defaults' as shown below.
4) You'll need to do this for each lens that is not automatically detected.
The correct lens profile should now be automatically assigned on import of the raw files.
Unfortunately this does not correct the issue of missing lens data used by LR for the Filter toolbar and Metadata panel.
That lens name does show up in the LR CC 2015.1 Filter Bar with the your DNG file on my Windows 7 system, but not in LR 5.7.1. I'm not sure why since the profile is supported in LR 5.7.

I have learned 2 things: Dropbox and automatic detection of the 28-45mm in develop module after saving as a new lens profile.
Because I have LR5.7.1, the problem in filter bar will continue. But I will try it in next time with LR 6, and I hope it will go like in your LC CC.
I can live with the only not recognized lens in filter bar, because it is allways the 28-45mm. All other my lenses (old smc645 and new HD645) will be recognised in filter bar. Hope that next new Pentax-645 lenses will be well recognized again.

The HD Pentax DA645 F4.5 28-45mm ED AW SR lens profile was apparently corrected in LR 6 and LR CC. The following lens info (bold) was added:
stCamera:LensInfo="280/10 450/10 45/10 45/10"
It's unlikely Adobe will correct the lens profile in LR5. You can download the LR6/CC updated profile here:
Rename the current profile with a .OLD extension in case you want to use later. Copy the above updated profile into the same 'Pentax' folder location (X = LR Version):
Applications/Adobe Photoshop Lightroom X/Right-click Show Package Contents/Resources/Lens Profiles
C:\Program Files\Adobe\Adobe Photoshop Lightroom X\Resources\LensProfiles\
If you reinstall LR5 or a LR5 update the new profile will need to be reinstalled manually. Keep a separate copy in your backup files.
Sorry the above procedure does not appear to work in LR5.7.1. With the updated lens profile the Filter bar still shows the lens as 'Unknown.' Adobe must have made other program changes in LR6/CC to correct the reading of Lens ID Info.
