If the files have different file names or different metadata? For example,they might have different dates attached? If there are multiple different metadata differences, Lightroom won't recognize the files as duplicates (even if the image data is exactly the same).
Would you please check if you have such images.
I've reviewed the import dates in Lightroom and there appears to be an error:
For example, when I hover over the photo that is found during import, it states my photo is from 14 years ago! Obviously, this is not the case and is likely why Lightroom insists on importing photos that are already part of my library. When I choose to import this photo and review the metadata, the same photo shows the correct date.

This EXACT problem constantly happens to me as well! I've spent many hours cleaning up duplicates, only to have them reappear. It would be nice if Adobe could the issue one and for all.
While importing make sure you are not using any preset, also make sure that your LR is up to date.

It was working fine for years... It never imported duplicates, but after a lightroom upgrade, it stoped identifying duplicates and want to import all pictures in the iphone, but also on any other camera
Sarika is correct! I import from the iPhone and change the names or add metadata in import and thus, the next time I do this, LR thinks ‘duplicates’ are not duplicates due to this difference. It can’t do otherwise! IF you import the iphone images and do absolutely nothing to alter them, the duplicate command should work. But few do this; we add information upon import or alter the files. Frustrating but I don’t know any kind of workaround and this has been an issue for me since day one. So not unexpected IMHO.
Hello. I too have noticed a change where now Lightroom 6 duplicate imports from Apple iPhones (iOS). The "Don't Import Suspected Duplicates" worked well for years. There are no Lightroom presets applied or other actions in place. Not sure if this issued started with an update to iOS 10.x or Adobe Lightroom 6.x? Still looking for a resolution to this...

I'm still having this exact same issue. My workflow is a bit different in that I still use image capture to download images off the iphone and into a folder on the computer. However, when I import images from that folder into LR, (even with the "don't import suspected duplicates") LR just goes right ahead and imports files that already exist in the catalog. I'm not using any import presets that I know of. I'm using an iPhone 6 and an Apple MacBook Pro running 10.10.5. Like some of you, I'm tired of manually deleting duplicate files. If anyone has any thoughts on how to avoid this issue, I'd love to hear about them. thanks, Joel
I have LR 6.10 on Windows 10. Same issue. too many duplicates import again including videos.
Lightroom seems to import images by the date and time modified not by capture date and time. This is the root of the problem in my opinion and why Adobe has not addressed it is very puzzling. It's a serious issue for an application designed for cataloguing images! Adobe, any comments? Am I missing something?
