I am actually not sure where thing is going wrong as Lightroom doesn't controls whether or not a file gets saved on the memory card.
please check some settings in the Camera.
So we're in some kind of limbo land between Adobe and Canon support because I've checked camera settings and have even called Canon support several times. I just called them again and read your response to them. They say there is nothing on the camera that determines whether the image saves on the card. They said this is being controlled by the software, as in this is a Lightroom issue.
It's not even a matter of whether or not it is possible for images to save to the memory card and also to the computer... it IS possible... that is how it was working for me for a year.
The only difference is that I have a new laptop. This is even the same version of Lightroom I was using on the previous computer.
It really is frustrating as the photographer to only have the image save to the computer because while the person at the computer can look at the image, when I go into playback mode on my camera it says the image is no longer available. Also if for some reason the connection to Lightroom fails, which does happen more frequently than I'd like, I end up losing images. Poof! Just gone. Before when the Lightroom tether would fail images were at least backed up on the card, and I could retrieve a shot if it could not be replicated later during the shoot.
Obviously this is a very important functional issue, not just a mere inconvenience. I've already lost important images in two different photo shoots due to this problem. If I can't get it solved then I'm going to have to stop using Lightroom to tether.
I thought Lightroom was trying to be the new industry standard for professional tethered shooting, and that is why I invested in the program. However I can't afford to use a software that loses images.
It was working so well for me for over a year -- I really wish I knew what went wrong.
What is the model of the camera you are using?
Have you performed any firmware upgrade to the camera?
I figured it out!!! I thought I would be a good citizen and report back on this issue. The problem was a DEFECTIVE MEMORY CARD. After troubleshooting with a Canon rep we actually tried tethering from the Canon EOS Utility 2 software which has a check box where you can tell it to save to both card + computer. Even when this was checked for some reason the image would not save to the memory card. We unplugged the tether and shot images on the camera without the cord plugged in and everything saved to the card normally. Then the support guy suggested I try shooting with a different memory card. After swapping out the memory card it worked great... saved in both places just like it was supposed to.
I'm so glad it was such a simple fix. Interesting that the memory card was otherwise functioning normally (for non-tethered shooting). Anyway, hopefully this can be helpful to someone else some day.
So GLAD to have this mystery solved!
It is the Canon Rebel T5i and I had not done any firmware updates.

Yes! It's now working in both LR and EOS Utility -- just the bad card. Thanks again for everyone's help.
