If you're choosing File > Export > Adobe PDF (Print), check "Bookmarks" at the bottom of the General panel.
Or, choose File > Export > Adobe PDF (Interactive).

Thank you, that worked. There is now a list of bookmarks in the PDF document, i.e., the bookmarks show in the bookmark navigation pane.
Unfortunately, the bookmarks do not work. I click on a bookmark, nothing happens. I RMB the bookmark, select "Go to Bookmark", nothing happens.
There is still something missing.

The part that I missed was that I unchecked the "Hyperlink" option. When the "Bookmarks" option is selected, "Hyperlinks" is also. I do not have any hyperlinks in my book so I unchecked it. Bad plan.
After enabling both "Bookmarks" and "Hyperlinks," bookmark selection in the PDF now takes me to the referenced page.
