I don't know what the problem is, but if you have placed an 'Autonumbering' on the master you wil get only one number on each page and not 4 different numbers. I don't really see what your point is?
Only the numbers at the bottom will change.
It looks to me like you've covered the numbers from the master page by adding content on the document page. Add another layer and put the master numbers on top.
i have the similar problem, i set up a multi-level list numbering in a document, it works in some chapters, but fails in another chapter, and in all these chapters i have the same configuration. Just as the screenshots show:
The issue exists on Heading 3, it works in chapters 1, 2,3, but fails in chapter 4. Here it should be 4.0.1 as chapters 1,2,3 numbered: . P.S: 4.2.1 inherits the same Heading 3 paragraph style without violation.
Can somebody comment on the wrong numbering? Many thanks!
Thanks for your comments. I still did not manage to solve the problem. As implied, I added an automatic number sequence to the Master (which reads 1 to 4) but on the actual pages for some reason it starts on "3" and the sequence does not continue in the next set of pages - it repeats itself. Any suggestions?
Please have in mind I'm quite new to Indesign, so I'm sure there is a easy way to solve the problem.
I think this is a common software bug, as it also exists in MS word.
From software perspective, numbering of Heading 3 is based on 1.2. (Heading 1 and Heading 2). When numbering of Heading 2 ascends in previous chapters, and users start a new chapter with the same Headings, Heading 3 still retrieves the ascended Heading 2 numbering, regardless of a new chapter where Heading 2 and Heading 3 should start from scratch .
If you want to re-present the bug, you can follow my configuration posted in the community and edit a document in your testing environment, then you will find the illogic.

Such numbers have nothing to do on the master. On the master should only be the page marker and it should be outside the margin guides, not inside. On the master shoul never be a text fram for the content except primary text frames, and these should be empty.
Content should only be added on the pages. Numbers have to be setup in Paragraph Styles.
