How do I create placeholder or ghost text in my fillable fields?

I have created a form with fillable boxes to enter the necessary information, but I would like to add text that shows in those boxes as an example to the user.  When the user clicks on the box to enter their required information, the text disappears and whatever text they write will remain.  I have the document in both a Word and PDF file and cannot fire out if what I am describing is possible.

Example (below) when the user clicks in the "name" field by moving thier cursor over the placehoder text and clicking on "John Hancock," it disappears and allows the user to enter thier name:

      NAME: John Hancock

      NAME: Connor Young
