Thank you for sharing your PDF document with us.
We confirmed that the problem is caused by a defect in Acrobat Reader for iOS version 15.3.0. Page contents such as hyperlink text and buttons in your PDF document are not rendered correctly due to the defect. Pages appear mostly blank/empty.
The defect has already been fixed in our internal build, which will be released to the App Store in a near future. (Unfortunately, I am not able to speculate on the exact release date.)
We will let you know when a new update becomes available in the App Store.

We just released Acrobat Reader for iOS version 15.4 on January 23, 2016.
This release includes the fix for the particular defect that you have encountered.
