The pages are not how I want them and will not print the way I want them.
It would help if you explain the difference between how you want them and how they preview/print.
I do suspect your pages are imposing differently than you expect because you actually need 2 more pages for a booklet. (Booklet imposition requires the number of pages to be a multiple of 4.) I believe you'd get the result you want if you add 2 blank pages to the file...either at the end, or before the "back cover" if your design includes one.
Pages are printing out of order. I have tried adding blank pages. I have tried selecting "print blank pages."
When I look at the print preview, it includes blank pages where they are needed and shows that it is correct, but then when I hit print it does not print as shown
Well again, I'm not sure you've offered enough detail on which to base specific help, but by your account of the procedure, it does sound as though InDesign is doing its part correctly, and there may be something in your printer setup that's crossing it up; paper size/orientation, duplexing options, etc.

- Use one of the Booklet-making scripts* for InDesign, to create a new "double-wide" InDesign doc that is properly imposed into Booklet order.
- Print the new Doc, or Export it to PDF.

There is a chance that you have created the proper printer's spreads. The problem is that Print Booklet assumes that you have not imposed them yourself. For example, if you manually created a spread with page one and page 16 (front and back cover), Print Booklet won't work. It assumes page one is first in pages, and page 16 is last in pages.
