How to create a checklist of form items which then goes to various bits of a pdf

I'm creating a form which at one point has a list of different insurance cover options on it. Depending on what the use clicks (they might click just one or all 5 so not a radio button set), they then get taken off to different sections of the pdf where they answer different questions. I don't know if it's possible to do something like this in InDesign. It almost requires a delayed action so that when they have selected their options, then click the forward button the form remembers the selection and takes them through the relevant parts of the form based on what they've selected in that initial checklist. Any ideas? It seems to be easy enough to sort it out if they just selected one option but the fact that there can be multiple options selected makes it hugely more complicated.

      Thanks for any advice!
