I am using Lightroom 6.0 / Standalone on a Mac.
Problem: Not possible to delete permanently (not only remove) a pic. from a catalog or folder. (and the HD !)
Under Photo menu > command + backslash, there do not appear (two) check boxes with_ remove or delete (permanently!) from the HD.
There appears only one checkbox with a remove command for not accepted pics….

This happens when the images are shown as missing in catalog , that is, the reference with the original image is broken.
This generally happens when the original image was moved/renamed/removed outside Lightroom.
In the case of missing photos, you only get an option to remove and not to delete as Lightroom does not know the location of your original image.
Opt + backspace immediately removes the file from the catalogue. That's what it is supposed to do. If you want to be given the choice of also removing the file from disk, you can just type backspace. And finally you can type Ctrl+Opt+Shift+Delete and the file will be removed both from the catalogue and from the disk, without giving you a chance to reconsider, so be careful.
