Need help to reactivate Lightroom 5 with my old serial number and TAN number. I got a license for Lightroom 5 when I bought my Leica delux 4 in 2012.

I got a license for Lightroom 5 when I bought my Leica delux 4 in 2012. It has worked great, but after I put an old hard disk to my PC (for 30 minutes - then I removed it) to get out some information from this old one, Lightroom now asks for license or try it for 30 days.  I have not installed Lightroom  on a new hard disk. It is still on the same hard disk. How can I now  get my Lightroom to run again using my serial and TAN number?? Need help to reactivate it with my old serial number and TAN number.

I use win 7.
