Check your InDesign version. Choose About InDesign (in the InDesign menu on a Mac, and I think the Help menu in Windows). It should say version 2015.2 (11.2).

Thank you! I suspected that the Indesign on my desktop isn't up to date, but that's what's weird. My version number on my inDesign says 2015.0 (11.0.0,72), but my Creative Cloud app doesn't indicate an update is needed. what's the dealio?
On my laptop... the publish online dialog looks exactly as you show it.
What operating system version?
It could be that you need to log in and log out from the Creative Cloud desktop app. Looks like you've not had the 2015.1 update in August or the 2015.2 update end of November.
If that doesn't do it, you might have to reinstall the Creative Cloud desktop app.

I'm in the process of uninstalling all the CC apps to uninstall and reinstall the CC app itself. Is a real bummer having to do this. blech.
I uninstalled everything, uninstalled the creative cloud desktop app, and reinstalled the CC desktop app and all the apps again. Got old versions again. ARGH! Even uninstalled and deleted the prefs... what is up?
You don't have to uninstall the apps to reinstall the Creative Cloud desktop app:
Usually you don't need to even uninstall the Creative Cloud app itself. Downloading will replace the desktop app with the new version. Here's the link:
