Could you please let me know the file format that you are trying to edit in Photoshop from Lightroom ?
What option are you selecting when doing the Edit in option for Lightroom , like " Edit a copy ", " Edit as original " or the other option ?
I've got LR configured to open the files as...
And, it wasn't working for either Edit a copy or Edit as original.
BTW, I went and closed everything, and re-opened LR 6.4 and the problem went away for a short time. But, it came back after editing a few other images.

Same issue. LR6.5 launches PS, but never opens the file to be edited.

Thank you for the update, I would request you to please uninstall and reinstall Photoshop once and then check.
That's not an acceptable troubleshooting step (unless Adobe would like to financially reimburse me for troubleshooting their product. My rates are posted here: Pricing - photography by db walton)
Older versions of LR aren't doing this. I suspect LR5 has a semaphore set in the code that isn't getting reset upon sensing the close of Photoshop (or it isn't sensing that Photoshop has been closed), and as a result, it "thinks" the image is open already. I saw this happen in an earlier version of LR... I want to say it was LR4.0 or 4.1.
Okay, here's something that might help you narrow it down...
When this happens, if I close LR6.5 completely, re-open it, and open a new catalog, the problem temporarily goes away.
