Can you clean it up via IDML export?

I'm wondering if I should just remove the logo in all places of the document to see if it also disappears in the odd locations?
Does it show up in your Links panel as being on those pages?
When you do Select All with the Selection tool on those pages, does it show up?
Do you have any hidden layers? Locked layers?

You could try using a Preflight rule to check for hidden items, something like this:
If you are using OSX this AppleScript would set the visibility of all graphics, pasted or linked, to visible:
tell application "Adobe InDesign CC 2014"
set visible of all graphics of active document to false
Again, I am grasping at straws. But put your cursor in each story on the page and go into the story editor. See if an anchored object shows up there.
Look in the Layers panel to make sure you get each story and object on the problem pages.
If that doesn't work, then try exporting half your document to PDF to see if it occurs, then half of that, then half of that. Repeat with the second half of the document. Try to isolate the problem page(s).

So, I actually deleted everything on the page (select all, delete) and exported it, and the logo still appears!
Check your Master pages. Is it there?
Can you share the ID file via Dropbox?

I finally figured it out. I deleted all logos and exported, and the problem disappeared. One by one I'm adding them back in to see which one was the troublemaker. I still find this strange, because those have been part of this file for 5 years. What changed, I do not know.
Thank you both for your continued ideas until I figured this out.
Oh, and it was on a master page, which I haven't edited in ages.
