I updated to from iphone 5 to iphone 6s last year. There were a number of documents stored on my adobe pdf reader on the previous phone. However, none of these documents appear on the app or online when I log in to my adobe account. How do I transfer these documents to the new phone? Do I need to sync something on my old phone, or transfer it onto the cloud?
What happens if I can't access my old iphone, are the documents lost forever?

If you were not logged-in on Adobe Reader DC and Mobile link (option to sync file on cloud account) wasn't ON. Most likely those files weren't uploaded to your cloud account. They were stored on phone storage only.
If you still have access to that phone, login with your Adobe id Acrobat Reader Help for iOS: Managing files and folders turn Mobile link on files will be synced to the cloud.
Then you can use same id to download file in new phone.
