Does this happen only with a particular PDF?
Check with other PDFs also if you face the same issue.
Try to repair the installation by navigating to Help>Repair installation .
Also let us know the reader version you have and the OS version on your system?

It is happening only in some parts of the pdf. I've tried deleting it and re-importing it and that was not effective. I am using the most up-to-date reader app. Using ios 9.3.2.
There is no option under help to repair the installation from within the app.
