      Can you generate a form in Fireworks?

          Yes. Try this...

          Draw a slice where you want the form. Change the slice type from Image
          to HTML. Click on the Edit button that appears in the Property
          inspector. The HTML editor box will open. Enter your form markup there.
          Let's say you wanted a drop down menu of months. You would enter the

          <select name="month">
          <option value="1">January</option>
          <option value="2">February</option>
          <option value="3">March</option>
          <option value="4">April</option>
          <option value="5">May</option>
          <option value="6">June</option>
          <option value="7">July</option>
          <option value="8">August</option>
          <option value="9">September</option>
          <option value="10">October</option>
          <option value="11">November</option>
          <option value="12">December</option>

          You won't see the form in Fireworks. You will in browser preview, however.

          Now, just because you can create one in Fireworks doesn't mean that you
          should. :-) Markup should be done in your HTML editor rather than in a
          graphics editor.
