Could be that you have applied a Character Style in the source (the title in the content)?

The problem starts between the Phase 1 and Phase two lines in the TOC.
I went into the document and see that Phase one title is set with a style that makes it bold and 14 pt.
That same style is used with the Phase two title. Something is still hiding somewhere. Here are screenshots from the pages where the text is originating. For some reason, when the TOC is being pulled together, something changes. at this point even though nothing is obvious.

You need to check that text for an applied Character STYLE, not just character formatting...
Local formating and character styles are getting cast over to the toc. If you have bold text and apply a character style that changes the characters to bold, you will not see a difference, but as the TOC generated input preseves local modifications, you will see them in your TOC.
The only possibility to avoid that is to clear local overrides.

Well, I managed to find and Clear the Character Style (at least it goes out of the menu as an option).
Then, the SAVE is not highlighted nor is the Update TOC highlighted so I am still missing something.
AND I am digging my hole deeper by somehow, I believe at the bottom of the styles menu doing something that has highlighted everything in green - and the edit/undo is not available to undo it.
At the top of your Paragraph Styles panel, click the + sign icon. It's showing local overrides on a paragraph style. The blue highlighting should go away.
That did get rid of the highlighting and let me save the file again.

I did as you suggested and have ultimately solved the problem with the bolding. The puzzle that I had to work through is the Update TOC would never light up for me after I made changes. Even when I would close the program and reopen it, the lines would remain bold. I finally opened the TOC window, clicked on a couple items (not changing any), saved that window, closed the file and reopened it again and now everything is fine YEA!

You should try getting rid of local overrides with applying styles (Paragraph styles, Character styles...).
