Ah, the joys of decapitation. A couple of possibilities:
- For the layer order for Megan, it looks like the neck is outside of the head or body group. Try putting it inside the body group as the top layer and see if that helps.
- Try removing the + from Face Background. That's telling CH that the Face should be its own free-moving part, which I'm not sure you want here if you want one connected head.
- Try deleting the origin in the PSD and just drag the origin point down for your head group inside CH instead. In Preview 4 you can now move/edit origins within CH, so there's not a need anymore to make them manually in Photoshop.

Just to follow up on this for anyone curious - we figured it out via DM. It was about taking the + out of the face background, letting it be one mesh instead of an independent part.
