On this page in the recources section of the fairly old doc from Adobe is a link to the profile creator software with a PDFs manual. Maybe this will be of help?

Thank you for the resource. I did a little more poking and found a higher-res version here:
The chart on the link you provided is fairly low res (33.3" x 18.75" at 144 ppi). The pdf available from the Cornell site is resolution-independent, and so can be printed at the output device's highest resolution. For example, a large format inkjet can print it at 2880 x 1440lpi (or ?), which is closer to the ultra-sharp glass-etched images that the pros use.
Alfred: If you have a newer version ISO calibration chart, please do share it. Apparently the ISO 12233 chart I found is somewhat outdated.
