Which version of InDesign are you using and are you exporting as a reflowable text ePub 3?

I've fudged it a bit by putting the title below the image - this keeps it on the same page - not very elegant.
You should be exporting to ePub 3.
Have you applied Paragraph and Character styles to all the text.
Have you anchored the image to the text?

I haven't anchored the text - no idea how to do that.
You anchor the image to where you want the image to appear in the text (see below, the little blue square on top of the pic – briefly, you drag this to the place in the text).
Producing Reflowable text ePubs is not intuitive, and quite different from producing print versions. I suggest you take an online video tutorial from TrainSimple.com or Lynda.com such as InDesign CC 2015: EPUB Fundamentals You can get a ten-day free trial with Lynda.com
