Two things to check:
the auto-play settings in your OS (not sure what Mac calls them)
the auto-play settings in the LR preferences dialog
There are no auto-play (or similar) settings in MacOS Sierra.
As I said in my original post: the settings in LR are correctly set to show the Import Dialog when a memory card is detected...
4 months later and no answer and even less a solution for this HUGE BUG in Lightroom. Adding all kinds of whistles and bells and idiotic gadgets for children and wanna-be photographers is apparently more important than allowing the BASIC FUNCTIONALITY of this program to ACTUALLY WORK.
A VERY DISAPPOINTED LIGHTROOM USER!!!
I noticed the same problem late last year, I posted here and got no replies at all. I don’t think it is going to be fixed any time soon.

Hey, I always turn off 'import automatically'. I like to be in control of what LR does and when it does it.
Bob... the issue is not "auto import" from the File menu; that's usually used for tethering, watched folders, etc. It is defaulted to "off" and is at least somewhat configurable. The issue being discussed here is the automatic launching of the Import Dialog Box for the user to Control and Specify the import process.
It's preferable to many that upon insertion of a memory card into an attached card-reader while Lightroom is open, that Lightroom's "Import Photos and Video" dialog box automatically appears offering its various Chooseable, Controlable import options... It no longer automatically opens when Lightroom recognizes a memory card, so importing now requires a few extra steps. For those of us shooting thousands of photos across multiple cards and/or catalogs, the extras add up. Being able to Control that process is precisely what this issue is all about.
Unfortunately, its return appears unlikely....

I insert a Camera SD card, Lightroom opens, the Import Dialog opens. All automatically.

Here are my settings in WIndows-10:
a) The Autoplay settings found in - [ Settings > Devices > Autoplay }
b) The option in Lightroom Preferences dialog:
Happy it works for you. The issue doesn't affect all users; some users (myself included) have even seen it come and go over time as ongoing updates are installed.
Again, as stated by the original poster, the Preferences settings are correct... hence this ongoing, unresolved issue.
Works for some, not others- a problem when you want it to work.
Do you use the Image Capture App? Its options may be set to auto open Lightroom.
