By any chance did "Layer 1 copy 5" have a layer mask in place before the action tried to add one. I it did it may  be the reason make layer mask is not and available option. That kind of error message is common error message one see when an action is used on document that structure is not what the action was recorded to process.  Action can not use logic to  analyze a document state.  Actions are just sequential steps. Step, step, step if a the if a document current state does  is not correct for the current action step the step will fail.

Single step through the action see where thing go wrong.

I wrote it look like that layer  has a layer mask. So make layer mask would not be an available option. Like the message states.
