    Messed up index when main index entry does not have a topic assigned

      My project uses conditional build tags to create various outputs. My top level index entries do not have topics assigned. I have done this purposefully so that the main index entry does not have an obnoxious amount of page numbers listed in the manuals I'm creating. All of my index subentries have topics assigned. I've found that my CHM output has incorrect index entries as a result. For example, it might take one index entry (e.g., data) and merge it with the the wrong sub entries (e.g. enter, delete). So the index should show:

      data
      name
      -enter
      -delete

      But instead it doesn't show the name entry and just shows:
      data
      -enter
      -delete

      Has anyone seen this?
