pictures randomly being sent to desktop in single files

Hey all of my pictures are being sent to my desktop in single file folders. I don't know if I clicked something in the options or what has happened. If anyone knows of any solution on how to fix this problem please let me know. Have been trying to fix it for the past month and still no luck.

Is this happening during Importing your photos, or when Exporting derivative files?

Can you show us a screen-clip of a sample area of the Folders with filenames shown.?

What 'fix' have you tried? Explain in more detail.
