Lightroom 4 Import Windows Disapeared

I've been using Lightroom 4 for the past few years and it's been working great. Then recently in the import window, I've lost some of the tabs for importing. Such as the file destination (see image below). Does anyone know how to fix this or had the same thing happen to them?

Mac or Windows, and EXACTLY which version of the operating system?
