Editing PDF Form: Changing format and adding lines for signatures

I'm working on a document for my job and I can't figure out on my version of Adobe Acrobat, how to change the formatting on this to where it looks like:

Date____________ % Completed ______________

Date _____________ % Completed ______________

[Test Screen Name]

The model is to re-edit the original, convert to PDF, and remake the form.
