Auto-linking page x-refs in Acrobat

I have a large PDF document containing many internal cross-references of the form (page 166) or (pages 166 to 167), plus index locators like 166 and 166-67.

Is there any way to auto link all of these page numbers to the correct page in the PDF? If it resulted in false positives where every 3-digit number in the book was linked, I could live with that. I do not have the original InDesign source files.

This can be done using a script or a stand-alone tool, and it's possible to limit it to only process numbers in a certain range, thus minimizing the number of false positives.
