Reinstall Acrobat (or run a Repair Installation from the Help menu), and make sure you update it to at least version 11.01.16.

Thank you, have version 11.0.22, have auto updates, did manual request anyway and still offers 11.0.22 only.
Also did repair anyway also with no success.
But did see Adobe not supporting XI beyond 15th October.
Have refrained from upgrading to DC, only offer as an annual subscription, as a single user do not use enough.
But would DC help. Still have outstanding forms use.
Any further solutions
Did you test the form with Acrobat Reader DC?
Thank you, but also fails, with form everything else still functions great but will not initiate Outlook 2016 open.
